Please complete the application in its entirety to be considered for the Shyolanda Brown Family Foundation Scholarship. All applications and documents must be submitted by April 7, 2022.

To complete the entire application, the following items must be uploaded:

Essay - A 1-page personal statement (at least 500 words) answering the following questions: 1) What impact has sports had on your life? 2) How will this scholarship help you? 3) What legacy do you hope to leave?

School Transcript - Your most recent school transcript is required. Attach proof of GPA.
Your Student Aid Report (SAR) - Upload your Student Aid Report (SAR). You can obtain your SAR by filling out your Free Application for Federal Student Aid (FAFSA) if you haven't already done so, at www.fafsa.gov.

I hereby understand that if chosen as a scholarship winner, according to Shyolanda Brown Family Foundation Scholarship policy, I must be present at any potential awards ceremony, surprise, or reception in May 2022 to receive my scholarship award. I hereby understand that if chosen as a scholarship winner, according to Shyolanda Brown Family Foundation Scholarship policy, it is my responsibility to remit to the Foundation the appropriate information for my scholarship to be paid directly to my educational institution for my second semester in December 2022/January 2023. I hereby understand I will not submit this application without all required attachments and supporting information. Incomplete applications or applications that do not meet eligibility criteria will not be considered for this scholarship.
